What is e-PAQ-online?
The e-PAQ® is a computerised system (electronic personal assessment questionnaire) that has been developed by a Sheffield Consultant Gynaecologist, Mr Stephen Radley, to help assess health, illness and quality of life. When you use e-PAQ-online, your results are private and anonymous. It takes 20-30 minutes to complete. When you have completed the questionnaire, you simply print out the results and take them to your Physiotherapy Appointment or e-mail your details as described below to allow your Physiotherapist to view your results prior to your appointment. You may delete your results afterwards if you wish.

How do I use the e-PAQ-online website?
If you have never used this web site before, don’t worry – the company have made it as easy as possible. Simply follow these 4 steps:

Step 1
Go to the e-PAQ-online website using the web address above. You will then be asked to REGISTER. This involves you entering your e-mail address (optional). You will need to create your own user name (this can be anything you want, as long as it’s 6 letters/characters or more. To keep your information private, don’t use your own name – think instead of a family pet, favourite place, random letters or numbers) and a password (this can be anything you want, as long as it’s 6 characters or more). Make a careful note of your user name and password and keep these in a diary or safe place.

Step 2
Click the START NEW QUESTIONNAIRE button on the computer screen. The questionnaire should start automatically and allow you to work through, question by question. The screen will appear larger and easier to use if you press the F11 button at the top of your keyboard, or use the ‘View/Full Screen’ function in the windows explorer toolbar on the screen. The questionnaire sometimes runs slowly, depending on the speed of the Internet. Please be patients and be prepared to take your time – about 20 minutes or so.

Step 3
After you have completed your questionnaire, you will have a number of options. Choose ‘View your complete results’. This will allow you to Print your results so that you can take them to your Physiotherapy Appointment. You can also keep a copy for your own use. Print both the ‘Analysis & Symptom Scores’ and the ‘Itemised Report’. The ‘Report Key’ can also be printed off and will help you better understand your results. Please make a note of your Questionnaire ID. This is shown at the top of each report page.

Step 4
To enable your Physiotherapist to access and evaluate your results prior to your appointment, please e-mail your user-name, password and Questionnaire ID along with your full name and date of birth to (insert e-mail address I can use at home and at clinic).

What if I can’t complete the questionnaire?

Don’t worry. As long as you have kept a note of your username and password, you can go back and find your record, print your results, resume the questionnaire or delete your answers at a later date. To do this, just go back to the www.epaq-online.co.uk web site and log again, using your original and password.
