When it comes to physiotherapy home exercise programs (HEPs), more isn’t always better. In fact, there’s growing evidence that giving patients too many exercises actually reduces the likelihood they’ll follow through with them.
Why Compliance Matters
Adherence to a prescribed physiotherapy exercise program is a key driver of successful recovery. Research consistently shows that many patients don’t complete their exercises as recommended — with estimates of non-adherence ranging from roughly half to two-thirds of all patients.
If exercises aren’t done, they can’t help. And that’s a major reason why many rehab plans don’t deliver the outcomes we expect.
The Tipping Point: How Many Is Too Many?
Most clinicians and evidence summaries agree:

1–3 exercises: often the sweet spot — manageable and memorable. Patients are more likely to understand and follow these.

More than 3–4 exercises: compliance drops noticeably. Overwhelming numbers, especially with complex movements, tend to discourage patients.

High lists (6–10+): many patients will do none reliably — because the plan feels too big to start.
One clinical perspective summarises this balance: prescribing no more than 3–4 targeted exercises per plan improves the chance people will actually do them, whereas long lists overwhelm and demotivate.

Led by physiotherapist Tanja Hayselden (MSc), who specialises in hormone-aware exercise, this class offers a safe, supportive space to rebuild strength, manage symptoms, and take control of your health.
Why Strength Training Matters During Menopause?
Menopause brings significant hormonal changes, particularly a drop in oestrogen, which affects bone density, muscle mass, metabolism, and cardiovascular health. These changes can lead to increased risk of osteoporosis, weight gain, fatigue, and reduced physical confidence.
But here’s the good news: evidence shows that strength training is one of the most effective ways to counteract these effects.

Laura graduated as a Physiotherapist from Sheffield Hallam University in 2008, initially started her career as a rotational Physiotherapist in the NHS before specialising in Neurology in 2011. In 2020 she became a team leader and was responsible for a large team of Physiotherapists and Occupational Therapists delivering NHS neurological rehabilitation services in North-East Derbyshire and Chesterfield community. Laura recently completed a master’s module on ‘Leading people and teams’ gaining a distinction.
Laura commented, “I am delighted to have been appointed as clinic manager at Hallamshire Physiotherapy and look forward to working alongside all the therapists and supporting patients in their rehabilitation, the clinic leads the way in the Sheffield area when it comes to treating neurological conditions and many other areas of treatment. Hallamshire Physiotherapy, Sheffield’s leading specialist physiotherapy clinic, announces the retirement of founder and director Karen Hodgson after 21 years of ownership.
The award-winning clinic was started by Steve and Karen Hodgson in 2004 with the intention of providing a centre of excellence for physiotherapy, based on outstanding staff and a commitment to provide a high-quality service. In 2022, Steve Hodgson retired as Director of the clinic and was replaced by Peter Thomason.
Today, Hallamshire Physiotherapy has over 20 Chartered Physiotherapists with clinics on Newbould Lane, Ecclesall Road and Clay Wheels lane and partnerships with over a dozen different Sheffield organisations.
A farewell party attended by staff and patients was organised last weekend to say thank you to Karen. Tributes were delivered by representatives of the MS Centre in Sheffield and Sheffield PD Warrior support groups for those with Parkinsons.
Karen in her speech to the guests said, “It was lovely to see so many come to say thank you. I only ever wanted to become a physio from the age of 12 and I have done it. I have thoroughly enjoyed my work, and I still enjoy my work, and hope to cover for therapists in the future. I know the clinic is in excellent hands with Peter Thomason and will continue to go from strength to strength. It’s been a pleasure and a privilege to either work with you, or be involved with your rehabilitation.”

Ian Brocklesby and Peter Thomason (Physiotherapists) will be running a FREE drop-in service for anyone attending Awesome Walls that day. They will be based upstairs in a private space.
To book your free Physiotherapy slot, simply write your name down on a time slot at reception on the day. Slots are unable to be reserved prior to the day.
You are then able to enjoy your climbing session, but please ensure you are waiting outside the room upstairs at the start of your time slot.
You will be called in to discuss your problem, before receiving advice/treatment.
We envisage having the capacity to see at least 24 people on the day. Each slot will be around 15-20 minutes depending on the demand.
You do not need to be a member to benefit from this, but you may want to make the most of your time by also fitting in a climb around the treatment. If you have not been climbing before but would like to learn, please contact the wall in advance to book in an induction session with one of the instructors.

At a recent party to celebrate the anniversary, Karen Hodgson, one of the original founders of the clinic announced her retirement and thanked all the staff and the wonderful clients she has treated over the years.
Twenty years ago, Steve and Karen Hodgson were teaching Physiotherapy at Sheffield Hallam and had an idea to deliver evidence-based treatment to clients from both an MSK and neurological client group. That was the seed to set up Hallamshire Physiotherapy. Quickly they found premises in Broomhill and started with three treatment rooms which within 4 years expanded to six rooms.

Hallamshire Physiotherapy Clinic has expanded since Covid with a gym facility on Ecclesall Road alongside an excellent working partnership with Community Wellness Services, and more recently in the North of the city in partnership with Sheffield Wednesday Community Foundation offering treatments, exercise classes and advice sessions.
Steve Hodgson retired from the clinic in 2022 and a new director Peter Thomason joined the team. Karen added, “Peter has been an excellent addition to the practice and is driving the ethos forward and continues to develop the excellent reputation of the clinic.”

Sheffield Wednesday Football Club Community Programme is providing FREE exercise classes and educational workshops to those suffering from chronic joint pain, and in partnership with Sheffield’s leading specialist physio clinic, Hallamshire Physiotherapy.
The project is predominantly aimed at men and women aged 50+ but not exclusively, to deliver an educational, self-management and coping process that involves using pacing activities, motivation and behaviour change methods to support people living with chronic joint pain, such as osteoarthritis of the knees and hips.
We took outcome measures before the classes started and then repeated the outcome measures after 3 months of attendance of the Osteoarthritis combined exercise and education classes.
The results showed a significant large improvement in outcomes of the members having only attended the classes for 3 months.
-> The average Oxford knee or hip score improved from 30 to 37.
-> 2 individuals were in the category of ‘it is highly likely you will require surgery’. After the 3 months, there were 0 in this category.
-> 7 were in the range of 20 to 29, just above the suggested surgery bracket. This reduced to only 2, suggesting that a further 5 potential surgeries may also be avoided!
-> considering the average joint replacement costs the NHS £13K, this is already a saving of £26K to the NHS and a potential further saving of £65K.
We will have further data in the coming months once some of the later starters have completed 3 months, but so far the results are looking incredible.

Women’s cricket has seen record-breaking numbers of teams established across the country in recent years, and Sheffield is no exception. To fund the development of girls’ cricket has been a challenge for many local clubs, but for one club, The BOTS, Hallamshire Physiotherapy in Sheffield were very pleased and proud to sponsor their team kit for the 2024 season.
Hallamshire Physiotherapy, Sheffield’s leading specialist physiotherapy clinic, are big advocates for sports, so were keen to help give these girls support to engage in cricket.
The BOTS girls cricket team was established in 2023 with members from the founding clubs Bradfield, Oughtibridge, Thorncliffe and Stocksbridge (hence BOTS) to enable the girls to play in the inaugural Sheffield and District Girls Hardball League.
Karen Hodgson, Director at Hallamshire Physiotherapy Clinic, said, “At Hallamshire Physiotherapy we believe that women should have more opportunity to participate in all sports. Personally, I am very proud to have been asked to sponsor the team as a female role model who is passionate about supporting girls in sport. We are honoured to have also received a signed ball from the girls.”
Emma Anderson from the BOTS, replied, “The girls were keen to have a team kit for the 2024 season which was provided thanks to the kind sponsorship by Hallamshire Physiotherapy. They voted for the design and wore their shirts with pride. Karen Hodgson has been very supportive of the team and the girls were delighted to show her their skills and meet her at a game as they participated in their first season in the Sheffield league.”

The backward chaining method is an evidence-based technique to learn how to get back up off the floor if you have fallen.
Falls are a major healthcare problem and can be devastating to the lives of the faller.
This is because over 30% of individuals over the age of 65 fall every year, and approximately 10% result in serious injuries including hip fractures or traumatic brain injuries (Appeadu and Bordoni 2023).
A fall can often result in a long lie on the floor before help can arrive, and therefore knowing how to get back up again can save a lot of distress, discomfort and improve outcomes.
We have successfully taught many older adults the backward chaining method, which has not only resulted in the ability to get back up off the floor, but also improving the confidence of older adults when on their feet, which in turn reduces the risk of falling.
Backward chaining breaks down the task into individual steps and is taught by learning the last stage of the exercise first before working your way back.

Start in a standing position. We recommend placing a pillow on the floor in front of the chair to make it more comfortable and help with mobility problems.
This can be removed after further practice.
Each step includes the step from above and ends by returning to the start position.
1. First, place hands onto the arms of the chair. This is to become comfortable with transferring your weight forwards.
2. Next, place one knee down onto the floor.
3. Place both knees on the floor into a kneeling position.
4. After kneeling, lower one side down, using your hand on the floor for support until you’re sitting.
5. Finally, bring your other hand back down to the floor.
We recommend that the method is initially performed with the help of a trained healthcare professional. This is because some individuals may be anxious and panic when practicing, and also reduces the risk of other problems.
